How to Order.

If you are a physician, there are a few ways to get started using FoundationOne or FoundationOne Heme in your clinical practice.

Getting Started

To order FoundationOne, FoundationOne Heme, or FoundationACT for a patient:

  1. Download and complete the appropriate Test Requisition Form. When applicable, include the contact information for the pathologist and institution where Foundation Medicine can retrieve the patient’s tumor specimen for analysis.
  2. Fax the requisition form, along with the pathology report and insurance information, to (617) 418-2290

Note:  If sending a solid tumor specimen, Foundation Medicine will complete the rest of the specimen retrieval process. We will contact the pathologist and send a Specimen Collection Kit with instructions for sample procurement and shipment.

If sending peripheral blood or bone marrow aspirate, please follow the specimen preparation instructions for liquid specimens.

FoundationOne

for solid tumors

All solid tumor types are acceptable. Please include the following items in the FoundationOne Specimen Kit prior to shipping to Foundation Medicine:

Easy-to-understand test results are available online or via fax in less than 14 days. Reports match cancer patients’ molecular information with targeted therapies, immunotherapies, and clinical trials to expand your treatment options.

FoundationOne Heme

for hematologic malignancies and sarcomas

FoundationOne Heme can be performed on FFPE, peripheral whole blood and bone marrow aspirate specimens. Please refer to the specimen guidelines for detailed instructions.

When submitting specimens to Foundation Medicine, please include the following items in the FoundationOne Heme Specimen Kit:

NOTE FOR BONE SAMPLES: Do not use strong acids to decalcify. Hydrochloric acid should be avoided; EDTA is recommended, and formic acid has mixed results. Place sample in decalcifying solution for minimal amount of time. Using a weaker acid and shorter time for decalcifying preserves the nucleic acid and increases likelihood for getting results on bone samples.

FoundationACT

FoundationACT requires two tubes of peripheral whole blood. Please refer to the Specimen Guidelines for detailed instructions.

Patient Reports

You may access reports and links to online resources through FoundationICE.

FoundationICE accounts are available for ordering physicians. Please contact your sales representative, visit our online sign-up page or call (888) 988-3639 to set up your account. Learn more about FoundationICE.

You may also receive your report as a PDF or fax. 

Billing

Foundation Medicine accepts all insurance plans; however, we are currently not in-network providers with most insurance plans. Payment plans are available through our Patient Assistance Program, and assistance may be available based on the patient's financial situation. Foundation Medicine is committed to making the billing and reimbursement process as straightforward as possible.

When you order FoundationOne, FoundationOne Heme, or FoundationACT we will work with you every step of the way:

  1. A physician orders any Foundation Medicine test and provides complete insurance information on the test requisition form. From here, the process is the same, regardless of the test ordered.
  2. Foundation Medicine performs the test and delivers the report to the physician. Results are typically returned to the physician within three weeks.
  3. Foundation Medicine bills the patient’s insurance company for the test, as the provider of service. Typically, it takes 60 to 90 days for the insurance company to respond to Foundation Medicine’s claims. During this time the patient may receive an Explanation of Benefits. This is not a bill and will explain the coverage initially offered by the insurance company for the test.
  4. If the insurance company denies coverage, Foundation Medicine works on behalf of the patient to attempt to obtain coverage and assists in pursuing appeals on the patient’s behalf to minimize the financial burden when appropriate. We may also contact your office for assistance in the appeals process.
  5. Foundation Medicine receives payment from the insurance company. Depending on the terms of the patient’s health care plan (or in the case of an uninsured patient), there may be financial responsibility for payment of any applicable co-payments, co-insurance, or deductible for the test, or payment for non-covered services. Foundation Medicine will not bill for the difference between the insurance’s allowed amount and the list price of the test.

Foundation Medicine will work directly with payers and patients. Depending on the terms of the patient’s health care plan (or in the case of an uninsured patient), there may be financial responsibility for payment of any applicable co-payments, co-insurance, or deductible for the test, or payment for non-covered services. Please contact our Client Services team for more information by calling (888) 988-3639 between 8:00 am ET and 8:00 pm ET, Monday through Friday, or send us an email.
